You will need a separate Entry Form for each entry.
Print as many copies as you need. Fill in, sign, and mail with your check and 4” x 6” photo to the Registrar.
The Entry Form can be filled in on your computer. You may type all your information on the form before printing.
To fill in before printing:
- Place your cursor on the line and type.
- Tab to the next line or move your cursor there.
- Fill in all information and print.
- For additional entries, change the specific blanks (quilt title, size, etc.) and print for each entry.
- Be sure to sign each Entry Form.
- You may wish to Save a copy of each Entry Form for your records. Give each form a different name.
